Apache :: All Of Sudden Server Error Log Started Outputting A Very Strange Message

I've had a server running for a year or so and all of the sudden the server error log started outputting a very strange message that i've never seen before:

'groups' is not recognized as an internal or external command, operable program or batch file.
From what I understand this means apache is trying to run a program that doesn't exist, what could be triggering this?
so after successfully installing and testing litespeed last night along on alternate port 1080 on my cpanel setup, I try to look at my cpanel's awstats this morning and:
Error: Couldn't open server log file "/usr/local/apache/domlogs/example.net" : Permission denied
(awstats has worked fine for 5 weeks now)
Googling for this shows other people with similar problem in the past but unfortunately no answers
I tried chmod -R 644 /usr/local/apache/domlogs and looks like the owners are still correct
I'm at a loss and really screwed if I can't easily fix soon... any ideas?
(as a side note, litespeed has stopped working properly too but I've killed it for now)
I've updated to Apache 2.4.10 from 2.4.9 and all of a sudden my pages don't load anymore.Previously I had this issue rarely and closing the PHP-cgi.exe processes fixed it.But since the update the issue happens almost constantly.
I was trying to set up a JBoss cluster with apache httpd mod cluster in windows.I was able to start the apache using the command 'httpd. exe'. But when I tried to access it using localhost:6666/mod_cluster_manager , the page was not accessible.I have the entry 127.0.0.1 localhost in my hosts file.
The http.conf contains lines as given below:

And now the problem. I want to automatically add extra html code with this filter. On most sites working fine without problem. PhpBB3 working just fine, for example.
But on other php scripts like vBulletin, SMF, Nuke, IPB it brokes entire site.
IE6 have unterminable refreshes, IE7 displays an error page, Firefox retunrs a blank page if extra code is placed on header. With Opera errors are minimal but sometimes it displays some strange code.
These errors are only related to vBulletin, SMF, Nuke, IPB and few others.
Removing this line $_=$_.'test';
everything is back to the normal without extra code
One of my servers stop generating server logs, ftp logs etc all of a sudden since yesterday. It is a windows IIS 6 server, anyone have any idea on how to repair?
We just upgraded to VBulletin 3.6.5, and are experiencing strange behavior. My forums are on a dedicated server, 27,000 users.
We'll be running along fine with loads of .5-2.00, etc. Then suddenly, the loads start climbing to 50, 70, 85+. We've been trying to figure out why. We even went on a different (much more robust) server, still the same result. This only started happening after the upgrade. Restarting the server corrects the problem, but only until it happens again. It can happen at anytime - during peak or off peak. The server may run for an hour or two until this happens, or it can happen 10 minutes after a restart.
Late last night when loads were normal we benched the server hard, and it ran just fine. We just can't figure out where this load spike is coming from.

I've got a few machines where Apache acts really strange and curious if anyone has any suggestions. I'd love to figure this out so it can actually be deployed to a larger amount of machines and not just test instances.
- Basic Information Apache 2.2.8 (Tried a few 2.2 versions) PHP 5.2.6 suPHP based
Prefork Based - Once a day at a random time Apache fails a request from remote monitoring. It comes back within a minute however is is inaccessible for that time. It sometimes gets picked up by 5 minute monitoring on the machine itself and it restarts the service obviously.
- PHP scripts fail to be killed at times resulting in memory being used. They need to be killed in order to go away.
Worker Based - Apache can stay up forever it does not fail any requests
- PHP scripts do not get killed at a more frequent basis than in prefork. You need to `kill -9 pid` in order to get rid of the php processes.
I read about very few issues with 2.2 so I'm quite confused by this.
Usually everything has been working fine with this .htaccess-file until today. There hasn't been any change on the system.
If you click on the hyperlink to the restricted area the login window appears where you need to type in your username and a password. After typing in these data the following message appears in the browser:
500 Internal Server Error
The server encountered an internal error or misconfigruation and was unable to complete your request. (…)
The apache error log file contains the following message:
Request exceeded the limit of 10 sub request nesting levels due to probable configuration error.
Use 'LimitInternalRecursion' to increase the limit if necessary. Use 'LogLevel debug' to get a backtrace.
The increase of the LimitInternalRecursion does not solve the problem. So I switched the LogLevel in the httpd.conf file from warn to debug and looked again on the entries of the error log.
The following appeared (I have replaced ip addresses, usernames and URL by xyz or <URL>) :

My server has been running fine for months and then I got this message:

The hostname (server.dealsreferals.com) resolves to . It should resolve to 69.46.27.111. Please be sure to correct /etc/hosts as well as the 'A' entry in zone file for the domain.
Some are all of these problems can be caused by /etc/resolv.conf being setup incorrectly. Please check this file if you believe everything else is correct.
You may be able to automaticly correct this problem by using the ' Add an A entry for your hostname ' under ' Dns Functions ' in your Web Host Manager
What is the problem? Maybe I should do what is suggested, however, why suddenly the problem?
My server is working fine. Should I just let this problem? What harm can happen?
I've only just finished setting up Apache, PHP and MySQL etc.
I went through the sitepoint pdf on how to set everything up - and i tested it by making a php page (showing the date) and saving it in the htdocs folder just like the instructions.
I go to view the page in firefox by going to [url] and i get this error message

my vBulletin forum members start complaing about an intermittent forbidden error message:
Forbidden You don't have permission to access /vb/forumdisplay.php on this server.
Additionally, a 403 Forbidden error was encountered while trying to use an ErrorDocument to handle the request
i did tail -f /usr/local/apache/logs/error_log and find lot of lines like this:

i also found the vb folder and all folders inside it have permession of 775 and all files have permession of 664
I kept getting the "friendly error" below even though i'd disabled it on the client browser (this prove that my client browser is not the problem). i purposely coded a simple script wrongly to get the detail error, but to no avail. I believe it has to do with the server setting.
Can you advise if there's any setting/configuration in the server that might cause this? Without the detail error, it will be impossible for me to debug what is wrong with my asp script.
Some of the stuffs that i'd checked on server. 1) The ISS has been set to send detail errors.
2) It is recognizing the .asp extension (approve) - else, the asp page wouldnt even work.
Been debugging this for few nights already, any help are very much appreciated.

I'm trying to create a new domain extranet.ondes.fr to have a forward on an another website.
