Mail Sending :: Warning: Increment Connection Ratelimit

One of my client getting error while sending email from outlook express. Can send email through webmail.
The error log getting is

2 computers, A and B
A's email address = bob@computerworld.com
B's email address = steve@computerworld.com
Both are setup to correctly SMTP AUTH using their respective email accounts.
Both are in Texas and Georgia, respectively, so they are *not* on the same network.
The problem? If Computer A sends 4 messages (or a message with 4 recipients) and gets rate limited, Computer B, that is very far away, has a separate email account all together, and a different IP address, also gets rate limited! Computer B cannot send an email at all.
I think for some reason Exim is limiting the entire domain, but it is not supposed to act that way. Why is it doing that, and more importantly, how can I stop it from doing that? I want it to limit per email address, not the entire domain.
I have tried with and without the $authenticated_id key. It doesn't seem to make a difference.
Here are the Exim logs showing the rejections:

i have this weird problem of emails not gettign delivered and i am hosting with theplanet. Every email is sitting on the queue and not delivered.
i checked with dnsreport and it says
Timed out [Last data sent: [Did not connect]]
and my exim_mainlog shows
R=dk_lookuphost defer (-1): host lookup did not complete
2008-06-03 04:49:59 SMTP command timeout on connection
i have restarted exim, checked firewall and still i am unable to fix my problem!
update:
i again checked with dnsreport and my nameservers are failing
NS FAIL NS A timeout occurred getting the NS records from your nameservers! None of your nameservers responded fast enough. They are probably down or unreachable. I can't continue since your nameservers aren't responding. If you have a Watchguard Firebox,
it's due to a bug in their DNS Proxy, which must be disabled (31 Jul 2006 UPDATE: several years after being informed of this, there is a rumor that there is a fix that allows the Watchguard DNS proxy to work).

When I send files to my through FTP, the server cuts connection and I get this message in messages log file
FAIL: ftp per_source_limit from xxx.xxx.xxx.xxx
xxx.... my IP
And I've observed the connection is closed and opened during the transfer.
I avoid disconnection adding per_source=unlimited in /etc/xinetd.d/ftp_psa
But I see this entry in the same message log
mod_delay/0.7: unable to open DelayTable '/var/proftpd.delay': No such file or directory

I moved over a 400mb ish forum database for VBB to another server and ran into
MySQL Error : Duplicate entry '0' for key 1
Which I imagine would have to do with auto incriment? What can I do about this? Is there a way I can bring it over and not hit this? Way to fix it? In the meantime all of the viewers cannot use the forum because I was forced to put it into maintenance mode so that no one can run into errors etc.

i am continously getting warning mail form cpanel cpu watch that my cpu got overloaded as like follows,

While processing, the cpu has been
maxed out for more than a 6 hour period. The current load/uptime line on the server at the time of
this email is
19:05:10 up 143 days, 20:38, 0 users, load average: 1.11, 0.69, 0.63
You should check the server to see why the load is so high and take
steps to lower the load. If you want stats to continue to run even with a high load; Edit
/var/cpanel/cpanel.config and change extracpus to a number larger then 0 (run
/usr/local/cpanel/startup afterwards to pickup the changes).
I searched on load average and found that average load for 1 cpu is 2 and default value is 5 to 10.but here i am getting mail for 1.11 itself.I changed the value of threshold of load average in stat and logs of tweak setting to 4.but no improvement.now also i am getting more than 4 mails for average load i.e 1.23 also
My questions are:
1)what is the criteria cpanel is checking for sending cpu-watch warning mail?
2)i am getting nothing more than rsync and exim at the time of overload(while using TOP command).So why now only i am getting warning mail?
what is the cause of this problem?
I was getting idea from internet to create account without stat/ log analyse function .But not sure how will it work and afraid of doing this.......
